Information required by law

This form is the internal reporting system of VZOR/SAMPLE under Slovak Act No. 54/2019 Coll. on the protection of whistleblowers. Reports are received and acknowledged by the law firm Haládik & Partners and investigated by the responsible person of the employer.

You can submit a report in writing through this form at any time, orally, or in person by appointment.

We will acknowledge receipt within 7 days and inform you of the outcome of the investigation within 90 days of the acknowledgement.

You can also report to the Whistleblower Protection Office (www.oznamovatelia.sk) or to the public prosecutor (www.genpro.gov.sk).

We will not disclose your identity to anyone without your written consent; you may remain anonymous. Information on whistleblower protection is provided by the Whistleblower Protection Office (www.oznamovatelia.sk).
